The film opens early on the morning of September 11, 2001 with the hijackers praying in their hotel room, then leaving for Newark Liberty International Airport. At the airport, the passengers and crew board United Airlines Flight 93 along with the hijackers. Shortly after boarding, Flight 93 runs into rush hour traffic, and the flight is momentarily delayed. The other 3 would-be-hijacked flights take off.

In Johannesburg, the small time criminal Tsotsi is a teenager without feelings, hardened by his tough life. After killing a man with his gang in a robbery; hitting the gangster Boston of his gang; humiliating a crippled beggar along one night, Tsotsi hijacks a car and under the despair of a woman, he shoots her in the stomach. While driving the car, Tsotsi finds that there is a baby on the back seat and the woman was a desperate mother. He brings the baby to his house in the slum and becomes attached to him. For six days, the baby changes his behavior, arousing and developing the sense of empathy and humanity in the cold blood killer.

Frank goes with some delinquents to the Blackwell Hotel, an abandoned place since a fire burnt the last two floors, with the purpose of cleaning the location, preparing it to work as a shelter for homeless; in return, the criminals will have their sentences reduced. During the night, the inmate Kira that has some Christian tattoos in her body is kidnapped by the deranged serial-killer Kane that collect the eyes of his victims, while the rest of the group is attacked by the psychopath with his ax.

In flashback scenes, Detective Eric Matthews (Donnie Wahlberg) escapes the bathroom he was left in Saw II by breaking his foot. He gets into a vicious fight with Amanda Young (Shawnee Smith), who eventually overcomes him and leaves him for dead. Before she leaves, Matthews leaves several angry parting comments, calling Amanda a “junkie bitch” and telling her she isn’t Jigsaw, causing her evident distress. Other flashbacks show that Amanda had been working with John Kramer, the Jigsaw Killer (Tobin Bell), prior to the events of the first film, and had been the one who actually instigated the abduction of Adam under Jigsaw’s orders.
